The short of the story is that your salvation is conditionally secure. If you hold fast to Jesus and don't deny and walk away from Him, noone and nothing can remove you from His hand.
As far as losing one's salvation, I see in scripture 3 ways to do so.
1 hardening one's heart by choosing to live in sin and refusing to repent
2 rejecting Jesus as the only means of salvation and turning to some other way to be saved such as turning to the Law of Moses the willful sin of Hebrews 10
3 teaching others a message that they don't need Jesus to get to God because all roads lead to God 1 Cor 3. defiling the temple of God by eliminating the foundation.
I highly recommend the series as it is very line-upon-line deep teaching showing what God's word says about the subject with all of the verses examined in context.
